Application Checklists
In addition to the electronic application submitted online, please make sure the following materials are received by Rackham and the Division of Kinesiology by January 15, 2008:
Step 1: To Rackham
- Electronic application
- Official transcripts from all colleges/universities attended. Mail these to:

Step 2: To the Division of Kinesiology
- Three (3) Letters of Recommendation from academic sources. These can be submitted as part of the electronic application process mentioned above, or hard copies can be sent to Kinesiology at the address below.
- Official transcripts from all colleges/universities attended
- Official GRE Scores ETS code 1839
- TOEFL Test of English as a Foreign Language (if applicable)
